Happy C.A.T. provides Trap-Neuter-Return (TNR) and colony management assistance. By helping unowned/free-roaming cats in need, you become part of the solution to the overpopulation crisis and improve the cats' lives. You will lessen the burden on overcrowded shelters and rescue groups.

BY SUBMITTING THIS FORM I AGREE TO ALL OF THE FOLLOWING REQUIRED SERVICES *The cat(s) I am presenting for Spay/Neuter are unowned / free roaming cats.I am responsible for claiming the cat(s) that I am presenting for Spay/Neuter and/or medical services. The cat(s) will be released back outside at the location where they were trapped. Releasing cats in a location other than where they were trapped is unlawful and will be reported to Indianapolis Animal Care services (Sec. 531-402 - Abandonment). A fine up to $2500 can be imposed for each violation of this section.Cat(s) will receive a mandatory ear tip, which serves as the permanent ID for a community cat and indicates the cat has been altered and vaccinated.Female cats will be spayed, regardless of the stage of pregnancy, and the pregnancy will be terminated.Happy C.A.T. will not handle additional medical services for unowned/freeroaming cats unless donations are received for this care. A donation to cover medical care is not required but greatly appreciated. We are a 501c3 non-profit and rely on donations and fundraising to fund our work.I have read and understand the colony guidelines, surgery and medical procedures this cat(s) will undergo and the policies guiding treatment listed on the back of the agreement form.

I have read and understand the colony guidelines, surgery and medical procedures this cat(s) will undergo and the policies guiding treatment listed on the back of the agreement form. *I recognize the risks cats face during handling, anesthesia, and surgery and hold Happy C.A.T. and its volunteers, Board of Directors, participating veterinarians and facilities' owners and lessees harmless should a cat presented experience complications, Injury, escape, or death. I acknowledge that any cat deemed by a participating veterinarian to be severely ill or injured will be humanely euthanized, per the clinic's policies.I release Happy C.A.T., its volunteers, its Board of Directors, participating veterinarians and facilities' owners and lessees from any liability for any injuries or property damage which I may Incur or cause while trapping, transporting, or caring for these cats.I understand that I must agree to and initial all conditions, services and terms stated above, and that Happy C.A.T. reserves the right to deny services to anyone who violates our policies and procedures.

Borrowed Traps *I acknowledge receipt of the following number humane trap(s). I will use the trap(s) provided to me only for services associated with the Happy C.A.T. TNR Program. I am responsible for returning these trap(s) to Happy C.A.T. within 30 days or I will be responsible for the replacement value of the trap ($75).
